Service Description

A follow-up is requested at least every 6-12 months to ensure you are not having adverse effects from any medication(s), and there have been no changes in your health if you have a chronic condition. Please schedule a follow-up if you do not get the results you desire to discuss alternative forms of treatment. Consult fee balance and any lab incurred during the visit are due at end of visit.
